3. Insurance referral, if required (may be listed on your insurance card)
4. Co-Payment. (We accept personal checks, cash and most major credit cards)
5. Medical Records, test results or other medical history forms
6. List of current medications, including over-the-counter drugs and vitamins.

When should I arrive?

Please arrive 15 minutes prior to your appointment to update your information and ensure that there are no delays to your appointment.

Cancellations

Please call us in advance to change or cancel your scheduled appointment. Cancellations within 2 business days of your appointment may result in a cancellation fee. Our providers are dedicated to ensuring that they can appropriately address all patient’s concerns and we appreciate the advance notice in cancellations.
